Description:

Per diem pharmacist responsibilities include providing pharmacy services to patients, physicians, and co-workers through accepted practices, policies, and legal requirements of federal and state regulations. The Pharmacist will provide clinical support services to support a full range of pharmacy operations. Central pharmacy activities includes reviewing patient profiles at the time of dispensing medications, ensuring prescription directions are appropriate, and other duties related to pharmacist services. As a decentralized Pharmacist, you will work collaboratively with physicians and nurses to provide care to the patient. Daily activities include drug therapy monitoring, provision of drug information and drug distribution activities. You must have strong IV, TPN, and compounding skills, and you must have a winning attitude. All candidates must have flexible hours, and be willing to work some weekends. Candidates with chemotherapy and anticoagulation skills are preferred.

Saint Francis Hospital is looking for a Full-time Clinical/Staff Pharmacist to join our team!

The Pharmacist is responsible for providing pharmacy services to patients, physicians, and colleagues through accepted practices, policies, and legal requirements of federal and state regulations. The pharmacist provides pharmaceutical care for neonatal through geriatric populations including developing safe, rational and cost-effective medication care plans to improve patient outcomes. Medication preparation and distribution is provided with a high degree of accuracy and under strict time-constraints. Daily activities include drug therapy monitoring, dosing and monitoring of vancomycin, aminoglycosides and warfarin, provision of drug information, drug distribution activities, medication education, and supervision of the technical staff. In the decentralized role the pharmacist collaboratives with physicians, nurses and other members of the interdisciplinary team to provide care to the patient. The ideal candidate must have strong clinical, IV, TPN, and compounding skills, strong attention to detail, knowledge of current evidence based pharmaceutical care and a winning attitude. Skills in the areas of chemotherapy, infectious diseases and anticoagulation are highly valued.

Routine schedule :

Pharmacists will work four 10 hour shifts per week on a predictable rotation:

  • week 1 - 6:30 am start - Main pharmacy coverage

  • week 2 and 3 - 8 am start - Decentralized coverage

  • week 4 - 10 am start - Main pharmacy coverage

Pharmacists cover every 4th weekend and rotating holidays.

This position will work be a hybrid clinical/staff position with the following breakdown (percentages are approximate):

  • 40% of shifts: Main pharmacy: Order verification, technician supervision, drug distribution oversight.

  • 40% of shifts: Decentralized coverage: patient education, medication reconciliation, rounding.

  • 20% of shifts: Quality improvement, policy/procedure writing/revision, education of pharmacy and non-pharmacy staff, MUE.

Minimum Qualifications:

  • A registered pharmacist from an accredited college of Pharmacy with a current Delaware license. Experience in hospital pharmacy practice or residency training is preferred.

  • Good communication skills.

  • Pharmacy Residency and PharmD desirable.

  • Strong human relations skills.

  • Basic computer skills.
