The Idaho Division of Veterans Services (IDVS) is recruiting for a full-time Social Worker for the brand-new state-of-the-art Veterans home in Post Falls!

The ideal candidate for this position will be relied on for providing professional social work services to elderly residents and their families.

    Responsibilities:

  • Provides professional social work/case management services to residents and families.

  • Conducts psychosocial assessments for the purpose of admission screening and evaluation of treatment and progress in the applicant's immediate milieu.

  • Participates in interdisciplinary team staffing to formulate treatment plans.

  • Provides individual casework counseling and evaluation services to implement specified treatment plans.

  • Provides intervention services to residents and their families throughout the residents' stay at the facility.

  • Communicates with relatives, guardians, and other appropriate persons regarding the resident's need for assistance in adjusting, modifying, or improving social functioning and behavior.

  • Investigates and resolves complaints made by or on behalf of the residents.

  • Represents residents on behalf of their claims, expenses, and their requests to the facility.

  • Inputs data with subsequent updates into the MDS Plan of Care for nursing care residents.

    Minimum Qualifications:

  • Licensure as a Social Worker in Idaho in accordance with Idaho Code 54-32.

    MQ Specialties:

  • Experience in a health care setting or facility social service.

    Desired Qualifications:

  • Knowledge of the Minimum Data Set (MDS) process.
